On the form for recertification it gives the address where the doctor, when he's filled out his portion, should send it. My doctor at GWU medical faculty associates sent it to the faxed address--"655 Virginia, Wash. DC" and that's where my doctor, after I filled out my portion, and he filled out his--sent it."
They are in the process of moving the Metro Accessibility Office. It is still at 300 7th St. SW, in the back of the building. But the address where the doctor sent my completed Recertification form for the red, white, and blue Card is 655 Virginia Ave., SW, which is the Lyft Office, I believe. But the address given for sending the Recertification form filled out is only that address on the form, whereas when I go in person, I am sent to the Metro Accessibility Office on 300 7th Street, back of building, SW.
So, when I tried to see what happened to the form that the doctor filled out containing what I filled out, too, before I gave it to him, I found myself in the Metro Accessibility Office on 300 7th St. SW. The Accessibility Office told me they had never received it; but they allowed me to leave my copy of the form that both my doctor and I filled out and he then faxed into 655 Virginia St., SW, which was listed for documents to be sent to on the form we filled out.
I've talked to management at the SOME building I live in and they said they'd look into my situation as much as they could.
That's how matters stand. I have no extension nor new picture card of the red, white and blue valid for extended use. But I did leave my copy of the form to which the clerk attached some data that said I am trying to get the red, white and blue Metro Accessibility Card. She told me it would take an additional 45 days.
UPDATE: 3/6/24: My sponsor, the FBI, has gotten in touch with me. I'm listed in their database. They say it may take going back to the DC Administrative Court; and they recommend my Case Manager appointed by Autumnleaf Health Care Agency get involved henceforth. I see my Case Manager on next Tuesday. She drops by, and I'll explain the situation about my recertification-application for the Red, White and Blue Metro Pass already submitted to Metro for action.
